Work in Progress

I am still working on getting this right. You can copy/paste but you will need to delete a line between category and title to get it on one page. But at least we have it! I can send this out as a word or pdf by email for those of you who want to have this completed before the meeting. I am sorry - WORK IN PROGRESS - =)
